colon cancer treatment - cure of advanced colon cancer possible. dr. anton titov, md: hello, we are with dr. heinz-josef lenz, who is professor of medicine and preventive medicine, and co-director of cancer center, and scientific director of cancer genetic unit at norris comprehensive cancer center, university of southern california. dr. lenz received his m.d. at johannes gutenberg university in mainz, germany. he completed a fellowship in oncology and hematology in tubingen, germany. dr. lenz published over 300 articles on oncology and molecular medicine and holds more than fifty patents on molecular markers in cancer. he's also an editor of the book "biomarkers in oncology," a cutting edge review of molecular markers in cancer. dr. lenz, hello and welcome! dr. heinz-josef lenz, md (colorectal cancer expert, california): welcome to you, too. thank you so much for inviting me to review colon cancer treatment and the status of colon cancer research. dr. anton titov, md: let's start with colorectal cancer treatment. that is treatment of colon cancer and treatment of rectal cancer. colon cancer therapy sees very rapid progress with introduction of new medications that target molecular pathways that are important in colorectal cancer. so avastin, erbitux, and other monoclonal antibodies that in combination with chemotherapeutic agents show high efficacy in colorectal cancer treatment. targeted therapies are particularly effective in metastatic colon cancer.


could you please talk about the recent progress in chemotherapy of colorectal cancer with particular emphasis on targeted therapies? dr. heinz-josef lenz, md (colorectal cancer expert, california): yeah, i think it's very important to realize that colon cancer is really different than any other metastatic solid tumors. we are living in the middle of a molecular revolution in treatment of colon cancer. before to diagnose and treat colon cancer we relied on the histology of tumor and staging with imaging technologies. now, we are classifying colorectal cancer tumors using molecular marker testing and genetic profiling of the tumor. based on this research and understanding many important molecular pathways in colon cancer, new medications have been developed to target the activated pathways in colon cancer. several molecular pathways play a significant role in the development of colon cancer and in progression of colon cancer to metastatic advanced disease. why is that so much more important for colon cancer compared to lung cancer or breast cancer? dr. heinz-josef lenz, md (colorectal cancer expert, california): because colon cancer has a very unique pattern of metastasis.


the colon cancer tumor usually metastasizes into the liver, and then maybe to the lung and other organs. but liver is one of the most fascinating organs in our body. because liver is the only organ which regenerates itself. now, what does that really mean? when colon cancer tumors are growing within the liver, and we can successfully shrink these metastatic tumors with chemotherapy. then we can remove the metastatic lesion from the liver and cure potentially the patient. cure is usually unheard of for metastatic cancer. so, increasing efficacy of our chemotherapeutic treatment options for patients with metastatic colorectal cancer, the goal of treatment today is "cure." this is particularly true if colon cancer metastases are limited to the liver. so goal of treatment is not only palliation, which was in the past. so the better understanding of the colon cancer genetic and molecular make-up helps us. understanding the choice of the colon cancer treatment, which ultimately can shrink and potentially allow curative resection of liver metastatic lesions makes a big difference for colon cancer patients. this unique characteristic of colon cancer needs to be recognized. colorectal cancer is a disease that is different than almost any other solid cancer tumor.
